The Old God Yogg-Saron can be engaged after the defeat of General Vezax in Ulduar. Entering his room will begin the encounter (unlike many bosses in Ulduar his aggro radius is massive). It is a three phase encounter, beginning with defeating Sara with her own guardians, then defeating the Brain of Yogg-Saron and finally defeating the body of Yogg-Saron. At the beginning of the encounter players will receive a debuff with 100 stacks called "Sanity". If this hits zero the player will have lost their mind and will to Yogg-Saron and will both be a slave to his will and see their entire raid as Faceless Ones.

Strategy
Phase 1
Phase 1 of the encounter begins with multiple green clouds rotating around the room in circles. These clouds spawn Guardians of Yogg-Saron naturally over time, from a random cloud. You can also force a spawn of a Guardian by running into the cloud, causing it glow and become active, spawning a Guardian a short time later.
Guardians of Yogg-Saron have a Dark Volley which inflicts a healing debuff to everyone they hit lasting a short duration, and heavy AOE damage. Dark Volley can and should be interrupted.
Guardians of Yogg-Saron explode for heavy AOE damage when they die, and the goal is to pull them on to Sara in the middle of the room when you kill them. Sara buffs the players with a small selection of abilities that both enhance and harm the players at the same time. Sara is normally not able to be attacked in this phase, and the only way to transition past the phase is by killing 8 Guardians on top of her, forcing her to take friendly fire damage.
Due to the long duration of the Yogg-Saron encounter, Heroism or Bloodlust can be used towards the end of Phase 1 to help you clean up any extra Guardians that will be spawned, and still allow you to use Heroism or Bloodlust again for the end of the fight.
When 8 guardians have been killed and damaged Sara, phase 2 will begin a few seconds later.
Phase 2
Immediately when phase 2 begins, multiple tentacle types spawn, including Crusher Tentacles, Corruptor Tentacles, and Constricting Tentacles. Crusher Tentacles, the largest of the bunch, channel Diminish Power on the raid, which reduces all damage dealt by 20%, for as long as they are untanked. Crusher Tentacles also gain a stacking buff increasing the damage they deal every time they are hit by a direct damage ability.
Thus the tanks on Crusher Tentacles will want to "joust" in and out of range of the Crusher Tentacles when the stacking buff is too high, and call for a brief halt in ranged DPS to allow the buff stack to fall off.
Constricting Tentacles randomly spawn under one person and immediately pick them up, preventing them from moving and using most abilities, while suffering damage over time. The tentacle must be killed to release the player.
Corruptor Tentacles have moderate health and repeatdly cast various dispellable abilities on the raid including Atrophy, a movement snare, a short duration Curse of Doom, and various mana and health draining and stunning poisons. All of the Corruptor Tentacles casts can be interrupted or they can be stunned to prevent casts. 1260x788
Portals open up around Yogg-Saron for a short time allowing players to click them and enter the brain realm. When entering, players will see one of three areas of World of Warcraft history, which Yogg-Saron is shown to have influenced. Players must kill the npcs in the area which are actually his tentacles, to gain access to his brain itself and damage it. The brain is channeling Induce Madness, which when finishes casting, causes all players still in the brain realm to instantly become insane and attack other players. Everyone must leave the brain realm before the cast finishes.
When players have cleared the path to Yogg-Saron's brain by killing the tentacles inside, all tentacles in the regular realm "outside" are stunned, and will cease all attacks and abilities. This is the "vulnerable" phase where you can maximize your DPS and actually harm Yogg-Saron.
Sanity is lost through various abilities Yogg-saron uses on you. You can regain your Sanity by standing in one of Freya's Sanity wells. You Sanity will replenish rapidly after a few seconds, up to a maximum of 100.
Brain Link is one of the main abilities to threaten your Sanity. A beam spawns between 2 players, linking them. The beam will be red if the players are too far from one another, suffering heavy damage and sanity loss every second. When they get closer, the beam is yellow and the damage and sanity loss goes away. Eventually the Brain Link fades from both players. Also, either player entering the portal realm will safely break the Brain Link.
Phase 3
When Yogg-Saron's brain is brought to 30%, a phase change happens. Tentacles will no longer be spawned, however, any remaining when the transition happens will persist. In this final phase, Immortal Guardians spawn about every 10 seconds.
Immortal Guardians have only Drain Life as an ability, which they channel on a random player and can be interrupted. They also spawn with several stacks of a buff that increases their size and damage done. As they suffer damage, they lose stacks of this buff, reducing their damage dealt and size.
These Guardians must be DPS'd down to low health, causing them to become vulnerable to Thorim's Titan Strike which kills them off. Othewise, these "immortal" guardians cannot be killed.
When an Immortal Guardian is low health, Yogg-Saron will begin to channel Empowering Shadows, a heal, on the Guardian. When the heal finishes, targetted Guardian and anything nearby (including other Immortal Guardians or Yogg-Saron himself) will be healed for a significant amount of health over time. The goal is to tank these away from Yogg-Saron so the heal will not be applied to him, and to kill them before the heal can be cast.
Yogg-Saron will use Lunatic Gaze in this phase, inflicting damage and sanity loss to everyone facing Yogg-Saron. The strategy is to tank things in such a way that people can attack them without facing Yogg-Saron, as well as not having the tanks face Yogg-Saron for extended periods, either. DPS on Yogg-Saron will have to face away when he channels Lunatic Gaze.
At some point, the tanks will likely have to restore their Sanity, which will mean the other tanks in the raid will have to cover all of the tanking of the Guardians, while the previous tank restores their Sanity.
Due to the 15 minute berserk, raids may have to offtank some amount of Immortal Guardians as they spawn while the raid ignores them, as they use bloodlust or heroism to burn down Yogg-Saron. The Guardians will have to be tanked away from Yogg-Saron to prevent a heal from going off and healing Yogg-Saron as well.
When Yogg-Saron reaches low health, Guardians despawn and he can be killed shortly after.
