Titles were introduced into World of Warcraft in the original release. However at that time, Players could only earn titles based off PvP rank from the old PvP combat system. These titles would change depending on your ranking on your realm so one week you could be a Knight and the next a Corporal if you didnt PvP to upkeep your tank.
With the addition of patch 2.3, titles became more widely used in The Burning Crusade. Players could now earn a title by completing quests or achievements in the game. The Burning Crusade titles were limited mainly to raiding achievements and Arena rankings.
The release of Wrath of the Lich King expanded on the title system more by adding a ton of new titles that could be earned not only in the Arena or by endgame raiding but also by general game play for things like reputation, world events, generic achievements, quests, etc. In addition WotLK also introduced some new title concepts for rewards in realm first achievements for killing raid bosses first on a specific realm. These titles are limited to the 25 versions of the encounters of those bosses and once earned can never be earned on that realm again.
